Teen Intensives Intake Form

Students will be signed out in the NBSS lobby by a staff member at the end of each day. If you allow your teen to sign themselves out, please check Yes below.

If they are not allowed to sign themselves out, only adults (persons signing out must be at least 18 years old) listed below will be permitted to sign the student out. A photo ID must be presented at time of pick up.

I understand and agree to the following terms regarding the use of my image, likeness, or voice in publicity and media materials by North Bennet Street School (NBSS). This consent release form applies to all forms of media, including but not limited to photographs, videos, audio recordings, and written materials.

• Consent and Authorization: I hereby grant NBSS, its representatives, employees, and authorized agents the right to use, reproduce, distribute, display, and publish my image, likeness, voice, and any other relevant personal information (such as my name and academic achievements) for the purpose of promoting and publicizing NBSS and its activities. This includes, but is not limited to, use in brochures, newsletters, website content, social media, press releases, advertisements, and other promotional materials.
• Permission for Minors (if applicable): If I am a parent or legal guardian providing consent on behalf of a minor student, I confirm that I have the authority to do so and agree to all the terms outlined here. I acknowledge that the same rights and releases as stated herein apply to the minor student.
• Revocability and Termination: I understand that this consent release is voluntary and can be revoked at any time by providing written notice to NBSS. However, I acknowledge that revocation may not affect materials that have already been published or distributed.
• Compensation: I agree that I will not receive any financial compensation, royalties, or other remuneration for the use of my image, likeness, voice, or personal information as outlined in this consent release.
• Release of Liability: I release NBSS, its representatives, employees, and authorized agents from any claims, demands, or causes of action arising out of or in connection with the use of my image, likeness, voice, or personal information, including any claims of defamation, invasion of privacy, or infringement of rights.
• Confidentiality: I understand that NBSS will make reasonable efforts to maintain the privacy and confidentiality of my personal information but cannot guarantee absolute confidentiality once materials are published or distributed.

I hereby release North Bennet Street School, its employees, instructors, and board members from any and all liability, cost, or expense associated with any injury that may occur while participating in the Teen Craft & Trade Intensives.

In addition, I understand and agree to the following:

• While NBSS takes reasonable precautions to ensure a safe and supportive learning environment, there are inherent risks associated with hands-on craft and trade work.
• Participants will be working with a variety of hand tools, sharp instruments, and powered equipment, under adult supervision.
• All instruction is led by adult instructors and staff members, all of whom have undergone background checks and are trained to provide appropriate supervision throughout the program.
• Participants may occasionally leave the NBSS facility in a group with staff and instructors for class-related activities or break time.
• All participants are expected to behave respectfully, follow safety rules, and take direction from instructors and staff. Unsafe, inappropriate, or disruptive behavior will not be tolerated and may result in dismissal from the program.
